About Us

Hospice of the East Bay (HEB) is a community-based program of home care for terminally ill patients. Care is given to patients and families by an interdisciplinary group consisting of physicians, registered nurses, home health aides, social workers, spiritual care coordinators, dieticians, pharmacists and patient support volunteers. Bereavement care is provided to the family for one year after the death. Hospice services are given regardless of the ability to pay, making the agency dependent on donations and insurance billing to meet operating expenses.

Position Summary

Reporting to the Vice President of Clinical Services & COO this leadership position is responsible for managing the Intake and Admissions department which includes Admission RN’s, Intake RN’s and the Admission/Intake support staff. This position is responsible for the daily supervision, implementation, delivery, coordination and supervision of the hospice services delivered to patients residing in nursing facilities and residential care facilities for the elderly by team members.

Essential Functions

Staff Supervision

  • Supervises and provides oversight and direction to the Intake and Admissions staff.
  • Oversees entries into the medical database, assuring accuracy, completeness and compliance with licensing regulations, certification standards, as well as legal and ethical standards.
  • Evaluates data on referrals and develops appropriate action plans as needed.
  • Ensures the day-to-day operations of the Intake and Admissions Department run efficiently and effectively.
  • Prepares performance evaluations according to the HEB policies and procedures.
  • Identifies opportunities to enhance and improve the functioning of the department including meeting productivity standards for all admission staff members.
  • Prepare monthly, quarterly and annual reports to reflect department activity.

Staff Performance

  • Provides excellent customer service to all callers.
  • Ensures all calls are responded to within 15 minutes.
  • Dispatch staff to explain Hospice guidelines and obtain consent signatures.
  • Ensures compliance with regulations regarding timeliness of admission
  • Makes external referrals as necessary.
  • Ensures compliance with all state and federal regulations, as well as accreditation standards
  • In collaboration with medical staff determines the appropriateness of acceptance of the patient for hospice services, and makes external referrals as necessary.
  • Provides direct nursing admission services to patients as required.

Administrative

  • Manages budget to ensure cost neutral or positive cash budget.
  • Manage department to ensure staffing meets demand; anticipates weekend/holiday needs.
  • Evaluates the staffing requirements and recommends changes in the staffing patterns.
  • Reviews and approves the daily time sheets.
  • Reviews and approves the time off requests for staff.
  • Recruits, screens and interviews candidates for open positions and participates in orientation of new personnel.
  • Participates in the establishment of department administrative policies and procedures.
  • Participates in the Quality Improvement and Utilization Review Activities.
  • Performs consultation visits to Home, Hospitals, ALF, CCRC and other settings
  • Participates in AOC rotation.
  • Participates in the development of the department’s annual goals and objectives and quarterly evaluation.
  • Rounds monthly and actively works on 90 day plan.
  • Supports agency goals.
  • Other duties as assigned.

Position Requirements

Strong leadership ability in a fast paced environment. Excellent interpersonal, communication and critical thinking skills. Ability to discern sensitive issues, respond appropriately and react quickly. Knowledge and understanding of the hospice philosophy and hospice criteria for admission and the admission process. Ability to work well under pressure, meeting multiple and sometimes competing deadlines, demands and changing priorities. Ability to coach and mentor others in order to develop a strong working team. Ability to work with Business Development team augment marketing plan. Strong service recovery ability.

Experience and Education

  • Current California registered nurse license required
  • Certified Hospice & Palliative Care Nurse preferred
  • BSN or MSN preferred
  • At least 2 years’ experience working as a professional nurse caring for the terminally ill
  • Prior management experience required, preferably in a hospice/home health care agency
  • CPR certification required
  • Excellent communication and listening skills required
  • California driver’s license and car in good working condition required
  • Excellent computer skills including proficiency in Microsoft Office (Word, Excel, PowerPoint and Outlook).
  • Ability to learn POC software
  • Experience working in a non-profit environment desirable.
  • Proven experience in providing quality clinical assessment and skills.
