Provides substance abuse counseling services within a primary care community health center clinic including, but not limited to, assessment, treatment planning, long-term counseling, education, case management, advocacy, and follow-up.
- Works with members of primary care team to assess, treat, and manage patients with chronic substance abuse issues within an integrated behavioral health care model. Team members include; primary care providers, Behavioral Health Consultants, and other care team members in patient evaluation and treatment to further sobriety.
- Performs intake, evaluation, orientation, treatment planning, and counseling, documenting within Epic EHR system.
- Teaches patients coping mechanisms.
- Attends and participates in Quality Improvement activities.
Skills & Knowledge
Has the competencies necessary to: Identify precipitating events; Gather histories of mental, emotional and physical disabilities, alcohol and drug use, and past mental health services; Assess family, social and work relationships; Conduct a mental status assessment; Document a multiaxial DSM diagnosis; Write and implement or supervise implementation of a treatment plan; Conduct and document a mental health assessment; Provide mental health treatment and rehabilitative services within ones scope of practice; Experience working with adult populations with addictions and complex issues. Execute evidence-based brief interventions. Ability to make quick and accurate assessments of mental health conditions related to substance abuse. Interest in gathering, tracking and presenting outcome data. Expertise in trauma-informed, culturally sensitive substance abuse and mental health treatment. Be knowledgeable of local and state resources. Be able to work with patients from infancy to geriatrics. Comfort with electronic medical records and charting. Flexible and able to assess and triage on the spot demands of medical staff.
