The Nurse Clinician (NC) provides care coordination services for pediatric patients and their families. The NC fosters effective communication among health care professionals who provide nursing care to children with altered health states. The NC develops and implements educational programs for patients, parents and families, hospital staff, and community health personnel.

Responsibilities

The Nurse Clinician implements and supports the philosophy, mission, values, standards, policies and procedures of Children’s Hospital and the Patient Care Services Division.

  1. Outreach to primary care physicians, specialty physicians, and other service providers to identify patients in need of care coordination.
  2. Works closely with families to guide them through the health care system to optimize the care and services they receive to improve child health, decrease costs, and improve satisfaction with care
  3. Practices the principles of Family Centered Care.
  4. Works closely with care coordinators based in other settings and levels of care to share responsibility in an efficient manner. Function as a resource to discharge planners in an inpatient setting as requested
  5. Travel within a 34 county area including home visiting, participation in primary and specialty care office visits, and collaborative meetings with community agencies including schools and county boards
  6. Ensuring the delivery of preventive services, health promotion activities and other interventions that proactively address potential risks
  7. Facilitates transitions between settings and caregivers
  8. Actively participates in policy and procedure development, continuing education, continuous quality improvement, research and/or evidenced based practice.
  9. Collaborates with program leaders to establish and/or maintain program goals and program budget as appropriate. Provides care in a fiscally responsible manner.
  10. Maintains accurate documentation in the medical record
  11. Participates in orientation and continuing education of staff and students as appropriate.
  12. Participates in the evaluation of peers for recognition and development.
  13. Maintains skills related to information technology requirements for the department.
  14. Develop patient and caregiver skills, knowledge and abilities to self-manage their own healthcare
  15. Refer, connect and collaborate with community agencies and resources to meet patient needs
  16. Performs medication reconciliation on enrollment, quarterly, and with changes
  17. Establishes and maintains collaboration of individualized care team with designated areas of responsibility and contact methods.
  18. Carries out specific care plan interventions as established by the care coordinator.
  19. Gathers and assess medical information (medications, diagnosis, immunizations) to complete a health risk assessment
  20. Demonstrates understanding of the principles of palliative care and incorporates those principles in care coordination activities
  21. Facilitates and coordinates quality patient care in a timely and cost-effective manner while promoting inter-disciplinary collaboration between all members of the health care team
  22. Assess readiness to change. Establishes and adjusts goals, interventions and care accordingly.
  23. Assists with troubleshooting medical issues between visits and hospitalizations
  24. Plan, provide, delegate and evaluate ongoing education needs of patients and families related to medical conditions and provider instructions.

Qualifications

Knowledge, skills and abilities required

  1. Current licensure as a registered nurse in the State of Ohio required.
  2. BSN required.
  3. Clinical skill and knowledge in the nursing care of children including normal growth and development and/or clinical expertise in specialized area required.
  4. Effective customer service, communication, and interpersonal skills required.
  5. Demonstrated leadership and teaching abilities required.
  6. Professionally appropriate analytical and organizational skills required.
  7. Current licensure as a registered nurse in the State of Ohio required.
  8. Travel and home visiting within a 34 county area expected.
  9. Good Driving record is required.
  10. Proof of auto insurance
  11. Active driver’s license

Minimum Physical Requirements

Must be able to see, speak, read and perform manual tasks with or without accommodation, and care for oneself with little or no difficulty. Performance of principal duties and responsibilities may require ability to travel.
