  • Monitors all incoming referrals to ensure they are being reviewed, assessed and scheduled in timely manner by enrollment and clinical teams
  • Determines clinically-appropriate level of care and applicable service provider based on clinical, demographic, and payer requirements
  • Maintains knowledgeable of all internal CHD programs and advocate internal referrals as appropriate using decision tree model
  • Develops and fosters good working relationships and ongoing communication and feedback with referral sources
  • Conducts outreach to established referral partnerships to maintain referral stream and client and referral satisfaction
  • Advocates and works to remove organizational obstacles for clients as they receive treatment
  • Provides leadership, supervision, and training to enrollment staff
  • Evaluates and assesses patterns of referrals to CHD outpatient clinics with continued feedback/discussion link to management, business development and marketing resources

Qualifications and requirements:

  • Master’s Degree in Human Services or related field, required
  • Must have at least two years' experience providing direct services to clients with serious mental illness, developmental disabilities, or substance use disorders or linking these individuals to services such as medical, legal, housing, financial services, psychiatric and educational
  • Knowledge of clinical specialties
  • Experience with intake and admission in behavioral health settings
  • Marketing/program development and outreach skills
  • Excellent customer service and communication skills
  • Bilingual capacity (Spanish/English) strongly preferred
  • Organized and efficient; detail oriented
  • Computer proficiency and experience with electronic health records
  • Knowledge of Medicaid/Medicare and other third party insurance coverages
  • Must have a valid driver’s license and vehicle available for work purposes
