Community Home Health and Hospice is seeking a dedicated and compassionate RN who possesses leadership and drive to support our Inpatient Units as a Hospice Assistant Clinical Coordinator. If you are someone who has experience in managing the day to day operations of an IPU and are looking to make an impact in the community, we want to hear from you!

*This position is Mon-Fri float between our Longview and Vancouver location.


The Inpatient Services Assistant Clinical Coordinator has primary responsibility for day-to-day inpatient care activities, staff scheduling and work flow. Ensures the quality of patient care through receiving referrals, planning, coordinating, implementing, and evaluating the service provided. Successful outcomes are demonstrated by:

  • High level staff and patient satisfaction scores
  • Positive financial outcomes
  • Exceptional communication, organizational skills and successful internal and external audit /survey and accreditation outcomes
  • Empowers and guides staff with managing workloads and scheduling, executing sound clinical judgment, preparing timely and adequate documentation
  • Is acutely keen at adhering to agency policy/procedure and compliance standards

Some of the key responsibilities include:

  • Responsible for supervision of Hospice Inpatient Administrative staff
  • Responsible for establishing and maintaining adequate staffing levels and scheduling to assure continuity of care
  • Reviews patient records with staff in order to verify, instruct, and assist in promoting effective, quality patient care and clinical documentation and timeliness
  • Demonstrates technical skills, teaching or other related services to staff or patients/families
  • Conducts and facilitates Interdisciplinary Team meetings on a regular basis
  • Monitors, reports and assists with evaluating effective orientation and ongoing training for staff
  • Works with hospital discharge planners, nurses and social workers to plan and organize services upon patients’ discharge to and from the inpatient unit
  • Acts as Hospice Supervisor On-Call on a rotating basis
  • Promotes a positive work environment for all staff and empowers members to participate in decision making


  • RN required. BSN preferred
  • Leadership experience 3 years preferred
  • Current WA and OR nursing licenses, current CPR certification
  • Knowledge of hospice regulations and third party reimbursement
  • Excellent organization and training skills
  • Valid driver’s license, reliable auto, and proof of current auto insurance
  • Proof of TB screening
  • Hospice Certification Preferred
  • Broad based knowledge of clinical nursing practice and medications including IV experience
