Overview

Full-time: Mon – Fri, 8 am - 5 pm

The RN or LPN facilitates mental health rehabilitation interventions and necessary supports to assist adults with mental illnesses to achieve their rehabilitative, resilience, and recovery goals. Conducts assessments and delivers services in clients’ natural environment. Provides nursing services in community setting and works with a dynamic interdisciplinary team.

Responsibilities

Works as an integral member of a team to support our clients in their recovery through individualized support, teaching, and case management.

Instructs individuals and families in health education and disease prevention; and coordinates care to patients utilizing the nursing processes of assessment, planning, intervention, implementation, and evaluation.

Visits clients in the community to determine client and family needs, develops plans to meet needs, and provides nursing services.

Works closely with consulting psychiatrist and medical doctors as assigned. This may include medication administration and training and coordination of care as part of the client’s treatment plan.

Qualifications

Unrestricted, valid Oregon Nursing license required. Experience working with mental health clients and addictions treatment preferred.

Candidate must be able to work independently and flexibly under general supervision. Computer proficiency required to use Electronic Health Records efficiently.

Must successfully complete state-required background and DMV checks, possess a valid OR driver's license, and demonstrate a safe driving record.
