Position Summary

Oversee professional nursing care to patients in a community health care setting. Supervise nursing support team, attendance, assign tasks, ensure completion of tasks in a timely manner, and assist nursing support team in providing patient care as needed. Provide directions for patient flow, and assign staff appropriately. Assist staff with clinical services as needed. Ensure clinical areas are appropriately stocked with medical and office supplies. Function as the lead nurse in coordinating care coordination activities with the multidisciplinary team, patients, and other staff of the health care team with regards to the primary care management of quality assurance indicators, patient flow, promoting wellness and educational activities in the management of patient diseases/condition within a primary care setting. Promotes patient-centered practice; supervise and assist with tracking and monitoring patients outcomes through our patient registry and electronic medical records system templates and generate weekly and monthly reports on outcomes, assuring quality care by adhering to therapeutic standards; measuring health outcome against patient care goals and standards; making or recommending necessary adjustments.

Job Duties and Responsibilities

Is responsible for effective and efficient nursing support care and patient flow

Ensure appropriate nursing support ratio to providers and exam room assignments

Ensure timely and high quality care is provided to patients by nursing support staff

Ensures patient care is delivered in a professional manner by all personnel

Records and investigates incidents reports in the clinical area pertaining to nursing and patient care.

Oversee the inventory control of nursing supplies in designated area

Effectively teaches families and patients

Regards safety of patients as a priority in performing duties

Handles patients in conformance with all established safety procedures

Ensure process is in place for administering medications as prescribed by physician, including refills

Recognizes patient conditions requiring immediate action and initiates appropriate life support measures when necessary.

Ensure patients/guest are treated in a professional and courteous manner

Ensure a process is in place for instructing patients on exam procedures and post procedures

Ensures a system is in place that allows you to be aware of and responsive to physical and psychological needs of the health center’s patients.

Oversee the maintenances of nursing equipment and that the clinical area remains in proper working condition at all times.

Keep equipment and work area neat and clean

Maintain an adequate inventory of all necessary supplies

Participate in continuing educational programs to maintain and upgrade nursing skills.

Participate in training new nursing support staff on the functions and the roles of the clinical nurse.

Under the directive of the Chief Medical Officer participate in quality improvement initiatives geared toward improving services to the patients and families.

Participates in continuing education programs, and provides in-services to the nursing and clerical staff.

Function as the lead nurse ensuring that the chronic care nurse coordinates care coordination activities with the multidisciplinary team, patients, and other staff of the health care team with regards to the primary care management of chronic disease

Provides comprehensive health care emphasizing healthy living, illness prevention, and individualized patient care plan specific to patient needs and physician practice preferences

Ensure a process is in place for coordinating patient care both on and off-site and staff understands the process and is carrying out the process according to our written procedures

Ensure a process is in place for scheduling and coordinating referrals to appropriate clinicians/services onsite and off-site specialist/services are kept and meeting the needs of each patients.

Ensure all appointments are scheduled in a timely manner

Initiate and contribute to the development of patient care plan, in collaboration with the patient, physician and other multidisciplinary team members.

Ensures a system is in place that regularly track, monitor and evaluate patient’s health outcome against patient care goals and standard through our patient registry and electronic medical records system templates and make or recommend necessary adjustment in patient care plan/objectives based on effectiveness of interventions and/or changes in condition or environment, in collaboration with the patient, physician and other multidisciplinary team members and generate weekly and monthly reports on outcomes.

Ensures appropriate systems are in place that provides screening, monitoring, and nursing assessment of patient’s physical and emotional well-being to multidisciplinary team

Ensures the chronic care nurse has a system in place that assists and support patients to follow through on prescribed medical interventions, diagnostic testing, and prescribed medication. Inform and educate patient regarding the meaning and implications of test results and prescribe interventions, as per clinic protocol

Ensures a system is in place that conducts individual and group educational sessions on and off site.

Attend and participate in community health fairs/events conducting health education and screening.

Ensures a system is in place that promotes patient’s independence by working with the multidisciplinary team to establish patient care goals; teaching patients, friend and family to understand condition, medications, and self-care skills; answering questions

Resolves patient problems and needs by utilizing multidisciplinary team strategies

Establishes a compassionated environment by providing emotional, psychological, and spiritual support to patients, friends and families

Acts as a central communication link between patients and the multidisciplinary team

Encourage maximum independence and accountability for self-care by providing the necessary education, demonstration, information to the patients.

Establish and maintain effective working and professional relationships across programs and services to further integrate and support coordinated patient service delivery.

Job Qualifications

Requires B.S. Degree in Nursing, Master’s Degree Preferred

Requires satisfactory completion of an accredited registered nurse program

Requires effective communication skills

Requires a minimum four (4) years’ experience; preferably related to primary care, community health nursing, and/or ambulatory care nursing. Emphasis on primary care services (prevention, intervention, and maintenance/support).

Requires a minimum six (6) years’ experience in supervision

Demonstrated clinical skill in physical examination and nursing intervention

Requires current CPR certification

Commitment to interdisciplinary teamwork

Requires a working knowledge of computers, keyboarding, Electronic Medical Records systems, and various programs (Excel, Word, PowerPoint, Outlook, etc.
