Primary Purpose:

Under direction of the Manager of Health Homes Program, the Health Home Program (HHP) Supervisor is responsible for managing the functions and activities associated with the Health Homes Program Department within Kern Health Systems (KHS). Performs a full range of Health Home Program administration and support functions such as oversight Of the HHP Advocates that are based off site in the CB-CME sites, track the Member incentive program and play a central role in partnering with the CB-CME sites.

Essential Functions:

  • Responsible for the general supervision of the HHP Advocates and HHP Navigators in the Health Home Program Department. Responsible for tracking and monitoring the staff’s monthly Productivity and ensure all documentation is being submitted on a timely manner
  • Responsible for overseeing the CB-CME sites’ outreach and referral process for the HHP members.
  • Responsible to track the performance outcome measures for the CB-CME sites.
  • Maintain communication and program coordination between KHS and CB-CME sites.
  • Assist and encourage team communication and leadership among staff/teams as acting liaison between staff and management
  • Provide guidance to the staff to understand their role in the HHP.
  • Assists in the interview and selection process for HHP Staff.
  • Maintains payroll time sheets, time off requests and completes annual evaluations for manager review.
  • Trains, develops and evaluates subordinate staff; provides input to Manager regarding disciplinary issues.
  • Maintains/updates training materials for new employees.
  • Responsible for following and enforcing all existing Kern Health Systems Health Homes Program policies and procedures.
  • Provide oversight and direction to the enrollment process as well as direct staff.
  • Maintain efficient processes for HHP across all KHS departments.
  • Conduct departmental operational meetings on a monthly and ad hoc basis.
  • Participates in the development and preparation of department budget, goals, and objectives consistent with organization’s goals.
  • Provides oversight of HHP operational and site reports.
  • Assists with oversight of member satisfaction surveying touch points and reports results; recommends actions to the Health Homes Manager.
  • Assists with routing of referrals from Case Management or PCP’s as needed.
  • Works collaboratively with Grievance department to investigate/resolve grievance issues.
  • Assists Operational Analyst (OA) with the eligibility/enrollment functions and works collaboratively with IT/OA to ensure that all enrollment/eligibility/inbound and outbound files are accurately and properly reflected in the systems.
  • Assist HHP sties with member issues and collaboration of care.
  • Perform job related duties as required.
  • Adhere to all company policies and procedures relative to employment and job responsibilities.

Employment Standards:

Education: Bachelor’s degree in healthcare or related field from an accredited institution or equivalent.

AND

Experience: Minimum of two (2) years’ experience managing the functions and activities associated with a home-based visitation Program AND five (5) years or more of supervisory or lead experience.

OR

Six (6) years’ experience managing the functions and activities associated with a home based visitation Program AND five (5) years or more of supervisory or lead experience required. (Education is substituted with experience on a year for year basis).

** Bilingual Spanish/English Required.

Knowledge of: Knowledge of the healthcare industry and familiarity with managed care plan designs and benefits.

Ability to: adapt to a rapidly evolving work environment; work independently; get along with co-workers; supervising multiple tasks and meet organizational deadlines; communicate effectively with staff, management and administration; prepare concise and comprehensive reports and effectively supervise and train staff. Must demonstrate excellent verbal and written communication skills.

Other: Required travel up to 20%. Possession of valid California Driver’s License and proof of valid State required auto liability insurance.
