Lower Umpqua Hospital District is seeking a full-time Pharmacy Director/Inpatient Pharmacist to join our team.

Position provides pharmaceutical assessment, treatment and care for the neonate, pediatric, adolescent, adult, and geriatric patient populations served. The pharmacist, proficient in the required competencies/skills, is responsible for monitoring drug therapy, providing drug information, consulting with medical, nursing and others to deliver high quality, cost effective pharmaceutical services to all Lower Umpqua Hospital District patients. The pharmacist directs the daily activities of pharmacy technicians and consistently demonstrates behaviors that model Lower Umpqua Hospital District’s core values and mission statement.


  1. Observe and enforce all aspects of pharmacy.
  2. Ensure that every medication is inspected for accuracy, completeness, safety (interactions, allergies, etc.) before leaving the pharmacy proper. This includes inspection of every drug in the medication cart before it is brought to the floor, every medication dispensed to ER, Ambulance Services, Radiology, Nuclear Med, Surgery, Outpatient Infusion, Physical/Occupational Therapy, Respiratory Therapy, and, as applicable, Extended Care Unit. This may not be an all-inclusive list; any and all medications that leave the pharmacy for ANY reason are to be inspected by a pharmacist before they leave the pharmacy.
    • Ensure that every medication has a label inclusive of all of the legal requirements. This can include lot and expiration numbers, descriptions of the medication inside the packaging (white, round, coated, any imprints), and methods and instructions of use. This SHALL include the intended recipient’s name, with the exception of either emergency meds being brought to the ER for immediate use, or medications that will be dispensed by a pharmacist to a dispensing nurse for immediate use by the patient.
    • Track all controlled substances, C-II through C-V. These shall be tracked in the customary manner; upon receipt drugs will be checked in using the invoice and, if a C-II, the form 222 used to order them. They will be logged into the log book, which shall be dated and signed. They will be logged out of the log book as used, and back counted to insure that the proper and correct inventory is maintained. Although a technician can perform these duties, a pharmacist must sign off that these have been performed accurately and completely.
    • Assume responsibility for all inspections that are required by law. Inspections that are due monthly include outdates for all areas where drugs are kept (surgery, ambulance, acute, etc), random narcotics inspections in all areas where drugs are kept except for inside the pharmacy, in which case a complete narcotics count must be performed monthly, med cart inspections regularly to insure all drugs are secure and in date, and refrigerator checks for temperature and outdating medications. These duties may be assigned to a technician, but a pharmacist must insure that they are performed and done correctly Carry out other duties as assigned.
    • Track medication errors
    • Steer Pharmacy and Therapeutics committee. This includes: Monitoring drug use trends throughout the facility and at local clinics to insure that meds needed are on hand, insuring that requested meds are cost effective and the use of such is evidence-based practice, monitoring of drug shortages, recalls, and format changes, new drug entity monitoring, etc.
    • Other duties as assigned


Education and License:

Doctor of Pharmacy degree
Valid Certified Oregon Pharmacist license required


CPR/BLS or ability to obtain within 30-days of hire.
ACLS or ability to obtain within 6-months of hire.


Clinical pharmacy experience in a comparable acute care setting preferred.

Additional Qualifications:

  1. Must have excellent interpersonal skills, as well as verbal and written communication skills necessary to communicate effectively with physicians, nurses, students, pharmacy personnel, patients, administrative and other hospital personnel.
  2. Must have a thorough knowledge of neonatal, pediatric, adult, and geriatric institutional pharmacy operating systems; Oregon and Federal pharmaceutical law.
  3. Must have the analytical skills necessary to coordinate all drug distribution and clinical activities.
  4. Must have work-related experiences necessary to consistently demonstrate an ability to keep up with patient workload: responding to changes in the workload as necessary to complete the job while coordinating the activities of pharmacy technicians
  5. Must have a high degree of independent problem resolution skills to effectively solve pharmacy related operational and clinical problems
  6. Must have data entry/computer skills
  7. Must have a working knowledge of pharmacokinetics, pharmacology, and toxicology as they relate to drug therapy.
  8. Must have the ability to concentrate and pay close attention to detail when reading and interpreting physician orders and related materials, and examining and closely inspecting materials when filing medication orders and preparing or checking IV solutions.
  9. Must be proficient in required competencies upon completion of orientation, as listed in the Pharmacy competency/skills inventory.
  10. Remain positive while dealing with complex processes and working with multiple providers and staff.
