Children and Families Program Manager

You can make a difference with the most vulnerable communities in San Francisco. San Francisco Health Plan (SFHP) is a community health plan providing affordable coverage to low income single adults and families residing in San Francisco.

The SFHP Children and Families Program Manager (CFPM) will be responsible for developing, monitoring and enhancing Care Management programs that support SFHP’s maternal, child, adolescent, and transitional aged-youth membership with specialized medical and behavioral health needs. This is fulfilled by developing collaborative partnerships with and acting as the primary point of contact for community organizations and entities that serve children, youth and families. The CFPM will ensure effective systems and processes are established between these entities focused on joint coordination of care and benefits for these members. In addition to working with our contracted partners and community agencies, the CFPM will work with internal SFHP staff and pediatric and perinatal clinical providers to make program recommendations to ensure quality of care for babies, children, adolescents and pregnant members.

The CFPM needs experience and comfort working with children and families in a variety of settings. The CFPM must have a working knowledge and passion for adolescent health, pediatric to adult transitions of care, family-centered care, children with special health care needs, and perinatal health. Strong communication and partnership-building skills are essential to this role. Experience with program development, program evaluation and process improvement is highly preferred. Knowledge of resources and community-based organizations serving the maternal, child and adolescent population in the City and County of San Francisco is ideal. This position will be based in the SFHP office and may involve travel within San Francisco County for meetings, conferences and trainings.

WHAT YOU WILL DO

  • Oversee development, implementation and process improvement of Care Management’s programs serving child and youth members and their caregivers/families (including the Enhanced Care Management (ECM) program under the CalAIM initiative)
  • Participate in and lead workgroups focused on addressing maternal, child, and adolescent access/ quality of care issues and health disparities (e.g. preterm birth, maternal mental health, preventative care, developmental screenings, Adverse Childhood Experiences, behavioral health treatment, Autism services etc.)
  • Continuously build and strengthen collaborative partnerships with the City and County, SFHP’s contracted pediatric and perinatal entities/providers, and community behavioral, social services, child welfare and school systems serving children, youth and families (conduct in-service presentations, attend workgroups etc.)
  • Facilitate, plan and actively participate in Golden Gate Regional Center and California Children Services joint meetings with SFHP.
  • Work with SFHP Compliance department to execute our contract requirements and policies related to maternal, child and family service, as well as support our contracted group’s adherence to pediatric-related care management contract requirements
  • Closely collaborate with the SFHP Behavioral Health Manager to design targeted interventions for improving access to pediatric and perinatal mental health services.
  • Strategic planning and project management, including development and implementation of work plans and metrics

WHAT YOU WILL BRING

  • MPH, MPA or MSW preferred or equivalent combination of education and experience
  • Ability to engage and work collaboratively with others, especially children, families and adolescents with specialized medical and behavioral health needs, and their care teams/providers
  • Health Plan experience a plus; Medi-Cal and/or Department of Health Care Services regulations and standards preferred
  • Experience as a Program or Project Manager and program development strongly preferred
  • Working experience with a population that includes those with mental illness, addiction issues, and/or homelessness
  • Ability to work with people from diverse backgrounds and experiences; cultural knowledge and sensitivity to the population being served; experience working with immigrant families preferred
  • Knowledge of San Francisco County community resources and pediatric/perinatal systems of care (medical, behavioral, substance use disorder, social welfare, and education)
