This role will provide program and people management for the Substance Use Disorder Program. The role will also provide substance use counseling, cognitive behavioral therapy, and cognitive enhancement therapy to Substance Use Disorder Programs patients within a primary care community health center clinic. Provide services including, but not limited to, assessment, treatment planning, long-term counseling, education, case management, advocacy, and follow-up.

  • Provide Substance Use Disorder program oversight, develop program components including but not limited to policies and procedures, key performance indicators, schedules and templates, community partnerships, training and education to best meet patient and organizational needs and in alignment with evidence-based practice and FQHC guidelines.
  • In partnership with the Behavioral Health Director, develop strategies and goals to support program effectiveness and growth across the system.
  • Manage performance of the team, conduct monthly meetups and provide real-time and consistent coaching for all direct reports.
  • Work with members of primary care team to assess, treat, and manage patients with chronic substance use disorder issues within an integrated behavioral health care model. Team members include; primary care providers, Behavioral Health Consultants, and other care team members in patient evaluation and treatment to further sobriety.

Skills & Knowledge

Has the competencies necessary to: Identify precipitating events; Gather histories of mental, emotional and physical disabilities, alcohol and drug use, and past mental health services; Assess family, social and work relationships; Conduct a mental status assessment; Document a multiaxial DSM diagnosis; Write and implement or supervise implementation of a treatment plan; Conduct and document a mental health assessment; Provide mental health treatment and rehabilitative services within ones scope of practice; Experience working with adult populations with addictions and complex issues. Execute evidence-based brief interventions. Ability to make quick and accurate assessments of mental health conditions related to substance use disorder. Interest in gathering, tracking and presenting outcome data. Expertise in trauma-informed, culturally sensitive substance use disorder and mental health treatment. Be knowledgeable of local and state resources. Be able to work with patients from infancy to geriatrics. Comfort with electronic medical records and charting. Flexible and able to assess and triage on the spot demands of medical staff.
