The Multipurpose Senior Services Program (MSSP) is a state Medi-Cal home and community-based waiver program serving low-income older adults who qualify for nursing home placement with the goal of connecting participants to the services needed to remain living at home. A nurse and social work care manager collaborate to jointly serve participants.
Under direct supervision of the Supervising Care Manager, the NCM is responsible for the evaluation of older adults as a total person and identifying the functional and health limitations that impede independent living. The program has three sites serving the six counties of El Dorado, Placer, Sacramento, Sutter, Yolo, and Yuba. Nurses are needed in two of the sites.
Please note, this position will require home visits in the homes of frail older adults as public health guidelines are lifted.
This position requires a valid current license to practice nursing in the State of California, and one year experience, geriatrics preferred. Compensation determined based on experience, education, and budget.
