The Multipurpose Senior Services Program is a Medi-Cal home and community-based services waiver program serving low-income frail older adults with the goal of assisting participants with remaining living safely at home. This program has three program sites serving six counties: El Dorado, Placer. Sacramento, Sutter, Yolo, and Yuba. Currently looking for 1 social work care manager for El Dorado County and a clinical social worker to potentially serve all three sites.


The SWCM conducts biopsychosocial assessments, develops care plans, and provides care management, care coordination and advocacy services for participants. Generally, home visits are required every 3 months for all participants, however during COVID-19 home visits have been placed temporarily on hold. All participants are contacted by phone a minimum on monthly. This position is heavy on paperwork and strong writing skills are required, which includes attention to detail, good spelling and grammar, and an ability to summarize information succinctly.


  1. Conducting in-depth assessments/reassessments covering psychosocial, rehabilitation, and environmental concerns.
  2. Referring clients to special consultants to provide detailed evaluations and recommendation to improve the client’s functional level.
  3. Participating in developing the care plan, implementing the services and conducting follow-up and monitoring of client’s needs and care plan.
  4. Identifying and developing support systems for the client.
  5. Consulting with the Nurse Care Manager.
  6. Care recording and reporting.
  7. Functioning as part of interdisciplinary team member.
  8. Attending trainings and conferences as appropriate to job duties or required by funding source(s).
  9. Community outreach and education activities.
  10. Other duties as assigned.


The SWCM position requires the minimum of a Bachelor’s degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, Sociology, or a related field and two years’ experience working directly with older adults; preferred is a MA degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, Sociology or a related field and one year of experience working with older adults.

Clinical SWCM must hold a valid clinical license in California or be in the process of completing clinical hours and licensure requirements.

Compensation determined based on experience, education, and budget.


The SWCM position requires attention to detail, good time management skills, ability to connect with older adults and their family members and caregivers, knowledge of community resources and how to locate resources, excellent writing skills, ability to structure time to meet all deadlines, and ability to diplomatically set boundaries when needed.
