Care Manager/ RN or LPN - Non-Exempt

The Care Manager (RN or LPN) is responsible for comprehensively developing, implementing, monitoring and evaluating individualized patient care plans for targeted patient populations. This position assesses needs, plans, coordinates and evaluates services of patients with the goal of equipping and empowering patients and their families to easily access resources and adopt healthy lifestyles that will increase their ability to remain healthy. Provides physical and psychological support to patients and families, and ensures the process incorporates evaluation of an evidence based plan of care with the utilization of all available resources. The Care Manager will assist Provider staff achieve desired patient outcomes by working collaboratively with providers, Quality Risk Director and other team members. This position functions within the scope of nursing practice as defined by the Arizona State Board of Nursing and Sunset Community Health Center’s policies and procedures and standards of professional practice to advocate for patient welfare and serves as a liaison between patients, their families, and healthcare providers. Also the care manager promotes and restores patient's health by ensuring completion of the nursing process: collaborating with providers and multidisciplinary team members.

Qualifications include: Degree from an accredited program as an RN or LPN. Licensed by the State of Arizona as an RN of LPN. At least two years’ experience in primary care ambulatory medicine or other nursing field with care management experience. Care manager certification preferred. Bilingual in English and Spanish. Must have the ability to interact with physicians and other healthcare professionals in a professional manner. Must have knowledge of physical and behavioral health medications and available community, local and regional social and/or health service agencies.

Must obtain a fingerprint clearance card and be able to successfully pass a background investigation. If not already obtained, Fingerprint Clearance card application must be submitted within 5 business days of employment.
