Hospice Aide - Certified Nursing Assistant (CNA)

We are currently looking for full-time Hospice Aides to assist our field hospice patients, Tuesday through Saturday, as well as work with our Tokarski Home patients as needed. When receiving hospice care in one's own home is not an option, Willamette Valley Hospice (WVH) offers the Edward F. Tokarski Home for our hospice patients who need a place to call home in their final months of life. The Home is a Class 3 adult foster home and is licensed by the state of Oregon. Individualized care is provided for up to five residents who are enrolled as patients of WVH.

The desired full-time candidate must be available 8:00 am to 5:00 pm Tuesday thru Saturday to provide care for our patients in their homes and care facilities. The candidate will also be required to be on an on-call rotation (approximately once every 5 weeks) with the Tokarski Home requiring on-call availability from Monday 11:00pm to Friday 11:00 pm.

Other requirements are as follows:

  • Provides personal care services to hospice patients under the supervision of the RN Case Manager
  • Assists in the development of individualized care plans as part of the hospice team
  • Responsible for electronic charting for patients
  • Assists with light housekeeping duties, as needed
  • Teaches and empowers family caregivers by providing information they need to care for their loved one confidently
  • Maintains established infection control and aseptic practices
  • Must relate to residents and families in an open, empathetic, and supportive manner while maintaining personal and professional boundaries.
  • Participates in periodic in-service programs and continuing education.
  • Completes the hours required by the state and DHS.
  • Keeps current with changes to policy and procedures.
  • Identifies safety issues and takes appropriate actions.
  • Recognizes emergencies and knows and institutes emergency procedures or notifies appropriate persons.
  • When necessary, provides personal care services to Tokarski Home residents, assist in the development of individualized care plans, medication management and charting for each resident, and assist with meal preparation and housekeeping.
  • Participates in staff meetings, committees, other focus groups, in-service education and orientation of other team members and volunteers.
  • Other duties as assigned.

Specific Job Skills

  • Ability to function in sensitive situations and manage multiple priorities.
  • Emotional maturity to cope with the ongoing needs of participation in a hospice program.
  • Knowledgeable about and supportive of the hospice philosophy.
  • Ability to lift and carry up to 50 pounds, push up to 200 pounds in a wheelchair, and pull up to 100 pounds.
  • Ability to understand, and carry out verbal and written English instructions, and communicate effectively with others in English both verbally and in writing.
  • Ability to continuously hear and speak in a manner understood by most people.
  • Ability to frequently bend, twist and kneel, frequently sit, stand, and make repetitive motions involving hands/wrists and feet involved in writing and driving.
  • Ability to climb stairs.


  • High school diploma or GED required.
  • 2 years of full-time experience (or equivalent) providing direct care to adults who are older or adults with physical disabilities and who require full assistance in four or more activities of daily living.
  • Completion of Adult Foster Home Caregiver training and test preferred.


  • Minimum of current Oregon CNA license- may have LPN.
  • Certified as Hospice Aide in Oregon and successfully pass agency administered test and skills checklist.
  • Must have current driver’s license, dependable transportation, and proof of current automobile insurance.
  • May be required to complete a physical capacity evaluation that indicates the ability to function under the specific job skills required for the position
