SUMMARY OF POSITION

The Monterey County Health Department’s Public Health Bureau is seeking to fill a vacant Public Health Licensed Vocational Nurse (LVN) position supporting the Whole Person Care/Enhanced Care Management (ECM) program. The ECM Public Health LVN functions under the supervision of the Supervising Public Health Nurse (SPHN) and the general direction of the Public Health Nurse (PHN) /Registered Nurse (RN) Case Manager. ECM program provides comprehensive case management services to high utilizers of hospital emergency and inpatient services, and individuals experiencing homelessness who have multiple chronic diseases, and/or behavioral health diagnosis. The program is a partnership with community organizations and primary care providers to help these individuals find a medical home, housing, social services, and mental health services. The LVN supports case management of these individuals by following the care plan, connecting the participants to resources, transporting and accompanying to health care services, providing educational support to manage chronic illness, and connect them to housing opportunities.

The Eligible List established by this recruitment process will be used to fill current and future vacancies, both regular and temporary, as they arise.

Working Knowledge of:

  • Principles, methods, and techniques used in clinical vocational nursing
  • Medical terminology
  • Basic math and statistical concepts
  • Basic computer knowledge

Some Knowledge of:

  • The principles, methods and techniques used in public health education
  • The principles, methods and techniques used in counseling designed to bring about lifestyle, nutritional, or other health behavior adjustments
  • The available sources and methods of obtaining public health, counseling, and medical treatment services
  • The principles of treatment, prevention, and infection control used in a specified medical specialty depending on the position being filled

Skill and Ability to:

  • Collect, tabulate and analyze data
  • Keep accurate records
  • Communicate effectively with persons from a variety of socioeconomic and cultural backgrounds for the purpose of gaining confidential medical and personal histories, and educating and counseling them concerning health practices or nutrition
  • Perform venipuncture and apply other vocational nursing skills
  • Develop and maintain effective work relationships with those contacted in the course of work
  • Work under limited daily supervision and function as part of a “team” approach to client management
  • Exhibit professionalism and deal tactfully with clients who have or who may be carrying a variety of diseases including the HIV Virus.
  • Speak before groups
  • Operate standard office equipment, including a computer and specialized software for case management.

EXAMPLES OF EDUCATION, EXPERIENCE AND TRAINING:

  • The knowledge, skills and abilities listed above may be acquired through various types of experience, education or training, typically:
  • Possession of a valid California license as a Vocational or Registered Nurse, and one year of experience as a community worker or related position, which included outreach education within a public health or voluntary non-profit health, related agency preferably dealing with ‘at risk’ populations.

Special Qualification:

  • Ability to communicate effectively in English and Spanish is required for these positions. Bilingual (English/Spanish) candidates must complete and receive a passing score on the bilingual oral and written examination prior to employment.

Conditions of Employment:

As a condition of employment, the incumbent will be required to:

  • Pursuant to Section 4211 of the California Business and Professions Code, incumbents in these positions must be licensed to perform venipuncture in California without a licensed physician in attendance and perform immunizations and/or medical tests under client specific orders from a physician or practitioner. This would require, at a minimum, licensure as a Licensed Vocational Nurse.
  • Health Department will conduct a thorough background and reference check process which includes a Department of Justice fingerprint check.
  • Must possess a valid California Class C driver’s license or be able to provide suitable transportation that is approved by the appointing authority.
  • May be required to work occasional evenings, weekends and holidays.
  • May be exposed to infectious respiratory secretions and bodily fluids.
