JOB SUMMARY/RESPONSIBILITIES:

  • Responsible for 24/7 operational management of case management across the organization.
  • Establishes and maintains a systematic review, assessment and evaluation of case management activities.
  • Provides support to the Manager in data collection, synthesis and analyses related to case management trends or issues.
  • Ensures case management activities support strategic plans, goals and objectives of QHS and are in compliance with all regulatory and accreditation requirements.
  • Provides assistance, guidance and training to staff on day to day operations and workflows.
  • Attends weekly length of stay and length of stay outlier department meetings to provide guidance to staff and escalation when required.
  • Consults with staff and provides guidance as required. Conducts patient care conferences when necessary with care coordination team, payors, physicians and multidisciplinary care team.
  • Escalates avoidable delays related to length of stay or medical necessity to appropriate department heads, medical directors, or physician advisor. Tracks avoidable delays and identifies trends.
  • Reviews monthly productivity reports for staff to track performance.
  • Monitors department(s) to ensure achievement of goals and objectives as established by the business plan, and alignment with the strategic plan and vision of QHS.

MINIMUM QUALIFICATIONS:

EDUCATION/CERTIFICATION AND LICENSURE:

  • Current Hawaii state license as a Registered Nurse.
  • Bachelor’s degree in Nursing.
  • Master’s degree preferred.

EXPERIENCE:

  • Three (3) years nursing experience in an acute care setting of which two (2) years case management experience in an acute care setting required.
  • One (1) year supervisory/leadership experience in an acute care or community setting.
  • Demonstrated knowledge of computer applications and hospital information systems required, preferably Epic. Bed management system knowledge preferred.
