Care Coordinator

JOB SUMMARY

The successful candidate will showcase an ability to interact therapeutically with patients, families, providers, co-workers and efficiently network with community agencies. The ability to maintain confidentiality, demonstrate high level organizational skills, and therapeutic communication skills is a must to succeed within this position. As a collaborator within the medical team, the Care Coordinator works closely with hospital providers, bedside nurses, primary care providers and associated clinical professionals to help patients in an acute setting -- either a hospital or another inpatient facility -- transition to a lower level of care. In practice, the Care Coordinator helps to remove barriers to the patient's timely discharge from a hospital or long-term acute-care environment. You will assist to coordinate care within the Observation, Inpatient, Emergency Room, and Swing Bed Rehabilitation Units. You will also serve a consultation role within the community to connect community members to medical and social resources when needed.

As a Care Coordinator you’ll monitor our patients hospital stay to ensure that optimal care is delivered in the most efficient and cost-effective manner. You will serve with continuity and assist to ensure care plan objectives remain strong, and all follow-up needs for patient are arranged during stay. Moreover, you’ll assist patients in arranging the care needed following a hospital stay. Planning and deliberation of patient recovery needs begin upon a patient’s admit to the hospital. You will serve a discharge planner role to collaborate with the patient, their family, the therapy team, to the clinical nursing staff delegate, and the provider to ensure that the patient’s discharge needs are identified and the patient is transitioned to the appropriate setting. The Care Coordinator can assist with providing information and referrals to community agencies, assist with transitions to skilled nursing facilities or long term care facilities, provide information for home care and local resource services, rehabilitative care, and out-patient medical treatment as well as providing assistance obtaining needed home medical equipment. The Care Coordinator will also serve as a community liaison to support patients with chronic illnesses and connect them to the appropriate area of community support as needed.

PRINCIPLE FUNCTIONS includes the following, other duties may be assigned:

  • Directly responsible to the CNO for outcomes as related to discharge planning and utilization review.
  • Performs duties as prescribed by the patient discharge care plan.
  • Provides information in response to queries from the public, doctors’ offices, families and outside facilities. Cleary communicate patient needs to the care team, serve as a patient advocate.
  • Maintains confidentiality when interacting with patients, families, personnel and the public.
  • Maintains compliance with State/Federal Guidelines and standards.
  • Conforms to all requirements of Medicare.
  • Attends weekly meeting with CNO regarding utilization review and discharge planning.
  • Lead the weekly ‘Swing Bed Rounds’ team/patient care summary huddles at bedside.
  • Form therapeutic and professional relationships with all admitted patients.
  • Demonstrates professional responsibility and strong integrity.
  • Complies with personnel policies and hospital safety policies.
  • Reports any patient care issues or barriers to the provider and CNO in a timely manner.
  • Conducts utilization review and discharge planning as scheduled or assigned by the CNO.
  • Participates in Continuing Education and other pertinent and appropriate learning experiences to maintain and increase personal and professional growth.
  • Utilizes work time appropriately to maximize productivity and enhance patient outcomes.
  • Completes and/or continues Discharge Planning Social assessment as quickly as possible from the time of admission for each patient. Completion of assessment includes interview with the patient, family or other caregivers, and also may utilize chart information from previous stays.
  • Participate with RN Management and Medical Director in the Swing Bed Administrative meetings to brainstorm the Swing Bed Rehabilitation program services.
  • Develop professional relationships with neighboring hospitals to promote the success of Swing Program.
  • Reviews and/or enters assessment using Interqual criteria and standards to complete review requirements as needed. Serve as the primary contact for program super-user and on call support with Providers.
  • Makes additional notes and documents conversations with other members of the interdisciplinary team on computer charting forms. Notes may also include conversations with caregivers, families, and other support systems involved with the care of the patient. Timely and productive note documentation is expected with this position.
  • Refers patients not currently requiring acute or skilled nursing care for discharge with appropriate services to lower level of care, or placement in appropriate facility. Preparation may include speaking with the patient, family, physicians, therapists, nurses, supervisors, intake coordinators, residential care facility managers, insurance companies, reviewers, etc.
  • Document all pertinent information regarding patient activity prior to discharge such as DME, Home Health agency referral, etc.
  • Educate patients upon discharge, delegate teaching needs to RN floor staff when needed.
  • Performs other duties as assigned or requested by the RN Manager and CNO.

The following are expectations of all Ferry County Health employees:

  • Attends a minimum of 80% of monthly department staff meetings throughout the year. Actively participates in monthly department staff meetings, to include active participation in setting department goals and supporting department functions to meet set goals.
  • Checks hospital email account a minimum of three (3) times per week.
  • Completes all mandatory education within given timeframe.
  • Adheres to all hospital, department, and safety policies, procedures and guidelines at all times.

JOB KNOWLEDGE & QUALIFICATIONS

Education

  • Active Licensed Practical Nurse or Registered Nurse degree or MSW.
  • Current DOH WA license.
  • Current BLS Certification.
  • Knowledge of medical terminology.

Training and Experience

  • Knowledge of medical terminology.
  • Adheres to dress code, appearance is neat and clean.
  • Completes annual education requirements.
  • Maintains regulatory requirements.
  • Maintains patient confidentiality at all times.
  • Reports to work on time and as scheduled, completes work within designated time.
  • Wears identification while on duty, uses computerized punch time system correctly.
  • Completes in-services and returns in a timely fashion.
  • Attends annual review and department in-services, as scheduled.
  • Attends scheduled staff meetings.
  • Represents the organization in a positive and professional manner in the community.
  • Establish relationships with surrounding facilities to promote referrals for Swing Bed needs.
  • Actively participates in performance improvement and quality improvement (QI) activities.
  • Complies with all organizational policies regarding ethical business practices.
  • Adheres to the mission, vision and values of the district, per the employee handbook.

Language Skills

Ability to read, analyze and interpret common scientific and technical journals and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.

Ability to apply mathematical operations to such tasks as frequency distribution, determination of test reliability and validity, analysis of variance, correlation techniques, sampling theory, and factor analysis.

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Strong ability to multi-task priorities of care between a multiple patient caseloads.

Wage: $69,000 to 81,000 DOE
