Job Description Summary

Access TLC is currently seeking a Home Health Clinical Nursing Supervisor to join our team. The Clinical Nursing Supervisor should be a self-motivated individual and will be responsible for ensuring that care and services are coordinated and delivered appropriately in accordance with agency standards. The Clinical Nursing Supervisor is responsible for the supervision of clinical personnel, assisting other managers with projects, and assist with the development of new programs.

Access TLC is consisting of hospice and home health agency that provides unparalleled quality of service that allows patients to have a better quality of life. We have been serving Ventura, Santa Barbara, and Los Angeles counties for 30 years.

This position is full-time and is based out of our San Fernando, CA office. Starting salary is $85,000/year - $90,000/year. Benefits include medical, dental, vision, chiropractic, and life insurance; medical and dental premiums are covered 100% for employees. Benefits also include paid vacation, sick and holidays.

Job Duties

  1. Receives case referrals. Reviews available patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate clinicians to case.
  2. Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs, and guides clinicians to promote more effective performance and delivery of quality home care services, and is always available during operating hours to assist clinicians as appropriate.
  3. Reviews patient’s clinical diagnosis, medications, procedures, and clinical course.
  4. Assists clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing plans of care.
  5. Attends case conference meetings with organization personnel to facilitate coordination of care.
  6. Conducts quarterly record reviews and communicates findings and recommendations to Clinical Managers and appropriate organization personnel.
  7. Assists in the screening and interviewing process of new organization personnel and makes recommendations for the employment of individuals. Assists in the orientation of new organization personnel.
  8. Assists the Clinical Managers in the planning, implementation, and evaluation of in-service and continuing education programs.
  9. Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
  10. Performs other duties and activities as delegated by the Clinical Director.


  1. Registered nurse with current licensure to practice professional nursing in the State of California.
  2. Home health experience.
  3. Knowledge and expertise in clinical decision-making, Quality Management review, and evaluation.
  4. Complies with accepted professional standards and practice.
  5. Complies and maintains current CPR certification.
  6. Demonstrates excellent observation, verbal, and written communication skills.
