Description

JOB SUMMARY: Responsible for providing nutritional care and education to multiple agency programs. Considers special dietary needs and food preferences of program participants. Coordinates with Food Service Manager and Nutrition program to ensure quality of meal services and that participant needs are being met.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Evaluates Redwood Coast PACE and Adult Day Health Day Center participant’s nutritional status, nutrient requirements, food tolerance, and resources for meal preparation. Conducts an initial nutrition assessment for new participants and performs reassessments as needed for current participants if their needs change.
  2. Develops and implements a nutritional Plan of Care with recommendations for nutritional supplements, meals, and/or home delivered meals. Reevaluates Plans of Care when a participant’s condition changes. Presents the Plan of Care to the Redwood Coast PACE Interdisciplinary Team.
  3. Recommends meal composition and dietary requirements for Redwood Coast PACE participants, coordinating with the Interdisciplinary Team and in compliance with the Primary Care Provider’s orders.
  4. Works closely with CarePartners to monitor participant eating habits. When appropriate, suggests modifications that best suit participant preferences, nutritional needs, and safety.
  5. Analyzes menus to meet all applicable regulations governing Title III – Older Americans Act (OAA) nutrition programs and Title 22 regulations governing service contracts.
  6. Provides guidance to the Food Service Manager in creating menus that are changed seasonally and works with the food service team on implementation.
  7. Active participant of the Redwood Coast PACE Interdisciplinary Team and the group’s meetings.
  8. Provides nutrition education, reviews training curriculums to ensure compliance, and creates and/or modifies educational materials to meet specific literacy needs for the following training areas:
    1. Redwood Coast PACE, Adult Day Health, and Kitchen Aide employees concerning special diets and modifications, and as required in consultation with Primary Care Providers and Speech Therapists.
    2. Redwood Coast PACE Interdisciplinary Team members.
    3. Annual training to meet Child and Adult Care Food Program (CACFP) regulations for Adult Day Health.
    4. Quarterly nutrition education to all Title III – Older Americans Act (OAA) participants, ensuring compliance with Area 1 Agency on Aging (A1AA) and the Title III – Older Americans Act (OAA) requirements.
    5. Redwood Coast PACE and Adult Day Health participants and family members/caregivers as ordered by the Primary Care Provider and in accordance with American Dietary Association standards.
  9. Participates in developing and monitoring policies and procedures as they relate to food service production and service.
  10. Monitors Redwood Coast PACE, Adult Day Health, and Nutrition program food facilities and kitchens for safe food handling and sanitation practices, as required by Title III – Older Americans Act (OAA) and in accordance with the California Retail Food Code (CRFC).
  11. Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to the Redwood Coast PACE Center Manager, Director of Health Services, Director of Nutrition & Activities and/or Food Services Manager. Takes necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors, and others.
  12. Consistently meets or exceeds targets for productivity.
  13. Continually seeks better ways for delivering services and communicating with participants.
  14. Consistently meets or exceeds customer service targets.
  15. Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality, and independence.

  19. Serves as a supervisor to student interns and volunteers working within the professional discipline and program area. Participates actively in their training as a guide, teacher, and mentor.

  21. Other duties as assigned.

Qualifications

EDUCATION &/or EXPERIENCE:

  1. Bachelor’s degree from an Academy of Nutrition and Dietetics accredited institution with a major in Nutrition, Master’s degree preferred.
  2. At least one (1) year of experience as a Dietitian working with a frail or elderly population.

LICENSES OR CERTIFICATES:

  1. Registered with the Commission on Dietetic Registration.
  2. Certified Diabetes Educator status preferred.
  3. Current CPR and First Aid certification within six (6) months of hire.
  4. Valid California Driver’s License, and personal auto liability insurance required. Must have a personal vehicle in good driving/operating condition, insured for State minimum liability requirements, and that can be used for the applicable job functions noted above.

PRE-SCREENING REQUIREMENTS:

  1. Requires clearance of a DOJ and FBI criminal history background check
  2. Pre-employment physical and PPD skin test.
  3. Requires Motor Vehicle Report verification.
  4. COVID-19 and Influenza (flu) vaccines preferred.

JOB SKILLS:

  1. Dietetic knowledge and skills necessary for delivering age-specific care and working with frail, elderly, or disabled participants in a clinical dietetic setting.
  2. Excellent interpersonal interactions that result in team building and contribute to effective work relationships, successful negotiation, and conflict resolution. Demonstrates effective social interaction with co-workers, management, participants, and community contacts including referral sources.
  3. Ability to work independently and within an Interdisciplinary Team and possession of strong organizational and time management skills.
  4. Ability to train and provide health education in the discipline of dietetics.
  5. Ability to complete duties within an agreed upon time frame and to adjust personal schedule if required, adapting appropriately to changes in priorities and/or workload and incorporating an understanding of detailed requirements into action plans and implementation.
  6. Demonstrates sound verbal and written communication skills to convey information effectively, including writing and developing menus.
  7. Maintains a positive attitude and balance in relationships with others in complex interpersonal situations.
  8. Ability to understand and incorporate detailed requirements in work activities. Ability to collect and analyze program data.
  9. Ability to think analytically and use good judgment, resourcefulness, flexibility, and problem-solving skills.
  10. Strong basic computer skills, particularly with Microsoft Outlook, Word, Power Point, and Excel, also including familiarity with electronic medical record software. Knowledge of web-based applications such as timekeeping, training, and email programs.

SUPERVISORY REQUIREMENTS:

  1. This position has no Supervisory requirements.

PHYSICAL REQUIREMENTS:

  1. Ability to sit at a desk and interact with a computer screen for extended periods of time.
  2. Hand and arm strength sufficient to operate a keyboard for several hours each day.
  3. Physical abilities sufficient to move between different staff work areas, communicate with co-workers and the public, operate a computer, produce reports, talk on the telephone, and travel to other agency worksites.
