Job Description

WES Health System provides compassionate care and services to our clients. We are seeking a dedicated Registered Nurse Case Manager to provide help to high-risk infants and children getting health care and developmental services. The Registered Nurse Case Manager will complete the following:

• promote optimal growth and development of infants.
• provide emotional support and relevant referrals to families for essential services.
• work with mothers to reduce stress and help explain identified medical conditions for further evaluation, diagnosis, and treatment.
• responsible for maintaining a caseload of clients by conducting outreach activities and utilizing the referral system from High-Risk Infant follow Up Program from the State of Illinois.

Requirements

• Bachelor’s degree in nursing from an Accredited College or University
• Active RN License in the state of Illinois
• 2 years of experience in Community Health or Maternal Child and Health nursing preferred
• New RNs are welcome to apply!
